Our primary care providers are here whether you need a checkup, immunizations, treatment for illnesses or ongoing care for health conditions. Our team will monitor and track your health over time, help you lead a healthy lifestyle and watch for issues or changes that need to be addressed. If your doctor determines that more specialized care is needed, we’ll make sure you’ll get it.

The role of our primary care team is to be your first point of contact whenever a health issue arises. Whenever you're feeling unwell, we encourage you to visit us instead of heading to the emergency room or urgent care. Primary care at Ochsner Community Health Centers makes preventing and managing chronic diseases easier in the long term.

Everyone! All locations offer comprehensive primary care for adults.

Please note pediatrics varies by location. For patients 65 and older, Ochsner 65 Plus offers customized primary care, nutrition and fitness coaching and community events.

Our goal is to be there for you when you’re under the weather and to help you be the healthiest you the rest of the time. From routine checkups and immunizations to health screenings and chronic disease management, our role is to be your first point of contact whenever a health issue arises.

We treat all patients regardless of their ability to pay, and our sliding-scale payment option ensures that everyone has access to quality care. We accept all insurance types, including Medicaid and Medicare.

Ochsner’s Community Health Workers help you navigate both healthcare and everyday challenges that can affect your well-being. They work closely with you to understand your needs and identify areas where you might need extra support — whether it's finding healthy food or reliable transportation. They’re here to help connect you with the right programs, services and resources so you can focus on staying healthy.

If you need support or if there’s been a change in your situation, talk to your primary care provider. They will refer you to a Community Health Worker to assess and address your needs.
