Ochsner’s skilled nursing facilities provide a high level of care for patients in Louisiana and Mississippi who no longer require inpatient hospital services but who are not yet ready to go home. Experienced registered nurses, physical therapists, speech therapists and occupational therapists provide medical and rehabilitation services to help patients recovering from a medical condition or surgery improve before they transition to home care.

Unlike many other skilled nursing facilities, hospitalists at Ochsner provide around-the-clock coverage, visiting patients in skilled nursing care daily. In addition, specialists, such as cardiologists and orthopedists, consult with patients, and 24-hour monitoring called telemetry is available for patients who require cardiac or other monitoring.

Ochsner’s dedicated skilled nursing facility, located in the Ochsner West Campus, features 30 private rooms, along with a large gym and activity room. With inpatient rehab services and long-term acute care at Ochsner Rehabilitation Hospital, also located at the Ochsner West Campus, patients can transition easily to skilled care after discharge from the hospital, all in the same state-of-the-art facility. Several other Ochsner locations also offer swing beds, which means they can transition from acute care (short-term treatment of a serious condition) status to skilled care status when needed. All skilled nursing units offer the same high level of patient care.

At Ochsner, we take a team approach to ensure every patient receives comprehensive, personalized care. Our skilled nursing care team includes:

  • Registered nurses

  • Nurse practitioners

  • A wound care nurse

  • Dietitians

  • Pharmacists

Patients also have access to physicians as needed.

A skilled nursing facility is designed for people who require short-term, not long-term, medical care or rehabilitation services after a hospital stay, while a nursing home provides high levels of assistance with the activities of daily living for people who can no longer perform them on their own.

Many patients qualify for skilled nursing, including those who:

  • Have had a recent surgery and require additional care to improve strength and wellness

  • Are debilitated or deconditioned (weak and unable to move properly) after a long hospital stay

  • Have newly inserted feeding tubes and would benefit from a focus on patient/family education

  • Need physical, occupational, speech or respiratory therapy

  • Require telemetry monitoring

  • Need long-term IV treatments, such as antibiotics or nutrition

  • Have had a stroke and require rehabilitation

The skilled nursing team offers many care options and can administer most medical services that are done in inpatient units (with the exception of the ICU). These include:
